Feedback

The Pathology Service at Sherwood Forest Hospitals NHS Foundation Trust is committed to providing a high quality diagnostic service supporting the clinical requirements of all users.

We are required by UKAS, our accreditation body, to carry out an annual survey to seek feedback from our users as to whether the Department meets their expectations and requirements. This feedback will help us shape the future of the service and help us to continuously improve.

We would be most grateful if you could spare a few moments to complete the below questionnaire. All responses will be anonymous.
