Completing an About Me form and taking it to your appointment will help your healthcare professional, and you, to make treatment decisions which are best for you.
Complete the form on Patient Knows Best (PKB) digital personal health record (external link), which is connected to the NHS App (external link), will be stored for future appointments.

The NHS App
The NHS App allows you to access a range of NHS services including ordering repeat prescriptions, viewing you health records and booking appointments. You can also access the same services in a web browser by logging in through the NHS website (external link).
Patient Knows Best
Patient Knows Best (PKB) is a single place to store your health information. It is linked to the NHS App and help to create personalised care plans and access to tools to monitor and track your health condition. Register for Patient Knows Best (external link).
Decision support tools
These are information booklets developed by the NHS (external link) to help inform you about your treatment options for some common musculoskeletal health conditions like knee and hip arthritis.
There are some similar support tools (external link) covering other conditions, like back and shoulder pain, developed by the Versus Arthritis group with support from Keele University and funding from the NHS.
