About Me form
Completing an About Me form and taking it to your appointment will help your healthcare professional, and you, to make treatment decisions which are best for you.
You can download the form from the Nottingham Integrated Care Board website.
Alternatively Complete the form on the Patients Know Best (PKB) website. This is a single place for all your health information and is connected to the NHS App so it will be stored for future appointments.

The OK to Ask campaign helps you think about your choices and ask questions, such as:
- What are all my options?
- What are the benefits and risks of each option?
- How will each option affect what is important to me?
- Are there any alternatives?
- What happens if I decide to do nothing?

The NHS App can allow you to access a range of NHS services including ordering repeat prescriptions, viewing you health records and booking appointments.

You can also access the same services in a web browser by logging in through the NHS website.
Patients Know Best (PKB) is a single place to store your health information. It is linked to the NHS App and helps to create personalised care plans and access to tools to monitor and track your health condition.
If you haven’t used PKB before, you can register through the NHS App. In the NHS App, go to Your Health then click Care Plans. You will see a button to Continue into PKB. Then you choose a new PKB password and set a security question. When you have done that, the app takes you into your PKB account. You don’t have to remember your PKB password because NHS App will remember it for you and take you straight into PKB the next time you log in.
