Getting the most our of your appointment

Completing an about me form (opens in new window) before your appointment and bringing it with you will help your healthcare professional and you to make treatment decisions which are best for you as an individual.
By the end your appointment you and your healthcare professional will be able to answer the following questions.
- What is my main problem?
- What do I need to do?
- Why is it important to do this?
You can complete the ‘About Me’ on paper, or on the Patient Knows Best (PKB) digital personal health record (external link) which is connected to the NHS App (external link). It will then be stored for future appointments. The NHS and Patient Knows Best APP also has a range of functions to help you manage your healthcare.
